Before you begin, please read this carefully and make sure you’re ready to complete the full application in one sitting. This form will take approximately one hour to complete.
🧾 What You’ll Need
To complete this application, you’ll need to have the following ready to go:

Your current and previous pastors

Two referees – one close friend, and one spiritual leader or pastor

📖 Your Testimony (Prepare Ahead)
You’ll be asked to reflect on your journey with God. Please consider writing your responses in advance and pasting them into the form.
You'll be asked to respond to these prompts:

What does your devotional life look like? How do you spend time with Jesus?
